Last night I was taken to Artermis Hospital in Gurgaon by ambulance with a fractured fibula.

The orthopedic doctor on duty (Dr Fahad) looked at the xray, consulted Dr Sandeep Chauhan who had seen the Xray and applied a plaster cast to my leg. He wrote out a prescription which is attached.

I had no where to go so I called some lawyers all of whom failed/ refused to provide any help with a place to stay.

I considered getting admitted in the hospital for the night or 2-3 nights to keep my leg immobilised.

I asked if an economy room was available. I was offered a twin room or the general ward which I refused.

While I was still deciding where I would go, a nurse came to my bed in the ER and started filling out some forms. I told her that I had not decided to get admitted. I asked her if she was filling out forms for me, but she denied it. She left the forms near my bed and went away.

I eventually decided to leave and stay in a hotel.

I looked at the forms the nurse was filling out. To my shock I found she had entered data for vital signs even though no vital signs were checked at the ER, because I was there for a fracture. I was not in great pain either.
I attach the form that the nurse filled out.

I apprehend that after Dr Fahad left, some persons including hospital staff attempted to fabricate my medical records once I indicated that I was considering staying in the hospital.

The Red Fox hotel and staff have admitted that a pesticide sprayer was on the corridor of the room I am in. They say he was spraying pesticides in adjacent rooms and in the shafts.

I do not think this is just a coincidence. This man had the opportunity to spray into my room under the door or through the AC shaft/ vents.

Pesticides/ chemicals were released in my room through the AC vents or under the door thrice since the morning.

I have now been assured by the hotel GM that no pesticides/ chemicals will be sprayed in the hotel during my stay.

Some chemicals (solvent and pesticides) are being released into my
room at Red Fox Hotel.

I am developing respiratory congestion, chest tightness,
breathlessness and labored breathing.


Late last night I checked into Red Fox Hotel in aerocity with my leg in a cast.

I slept fine and was alright till this morning.

Since this morning I have had 2 coughing fits. I did not cough all of
yesterday or last night.

I apprehend that some chemical could have been released into my room
this morning which made me cough.

I need police protection.

>> comply with the protection orders issued by this Court; and that I am
